The debuff is supposed to work only on PvP, not in pve or out of combat. Cuz even healing rotation in orgrim out of cb get half.

Sometimes healing spells heal 50% of their normal amount (ex: holy Light heals 16k instead of 33k), it occurs randomly and I can't heal at all. 3.
