2. The flower who is supposed to appear on your dead body is smaller than it should.

 

3. It is supposed to be bigger, atleast big enough to be seen without zooming in to your char to see it. Check wowhead link on 1. screenshot section on bottom right to see it's true size. Roughly said, it should be 1/3 of your body size.
