Разрывной дамажит 1 раз при юзе и потом еще 2 раза тики. Смотрите сами в логах, там где написано "попадение" - это он юзнул разрывной. А где написано "урон" - это тики. 01:25:21 - попадение 01:25:29 - попадение (прошло 8 сек, т.е. кд уже сбросилось на разрывной) После он юзнул черную стрелу (могла прокнуть пассивка). В таких случаях только видео записывать, потому что по логам очень сложно найти какой-то баг, мы ведь не знаем, прокнула у него пассивка или нет. Отклонено.

Looks like visual bug with spectator. When he use a Travel form, it also appears twice in the buffs. I'll check him in game.

Yes, but there is two spells reflected (incinerate and chaos bolt). It's a weird that incinerate hit for 49k and chaos bolt 57k, they are never so close in damage (especially with dmg procs). So, maybe spell reflection should reflects a full damage of spells, except chaos bolt? I found a videos with shadowburn reflect and seems like it's 1 to 1 damage or close to that. (5.0.5 patch, ~129k shadowburn) And another one: (5.4.x patch, probably prideful gear) As we see there in combat log, reflected shadowburn hit for ~200k.
